There is a specific form required - "Request for a Certificate of Eligibility." It is also known as VA Form 26-1880. You'll need to complete this form and send it to the VA Eligibility Center in North Carolina along with a copy of your DD-214 Member Copy 4. You can either obtain the form through your lender, your local VA Administration office, or by copying and pasting this link "http://www.vba.va.gov/pubs/forms/vba-26-1880-are.pdf" into your web browser. You'll find the instructions attached for your assistance. Plan on receiving it 45-60 days after application submission. There is an online system, as well, but it doesn't give as consistent results as good old-fashioned paper.
