Removing someone from ownership (title or deed) of a home is easy. The document to do so is called a "Quit Claim" Deed. Most commonly used because of divorce. You can also you the same form to add someone to ownership, which is commonly used to add adult children to ownership of the parents home. NOTE: Removing someone from ownership DOES NOT remove them from any responsibility to pay for the home if they are also on the mortgage loan.
