Government loans have a Streamline option that significantly reduces the documentation you need. IF your loan needs do not include a streamline, then typically the lender is going to need income documents to support your ability to repay, copy of current mortgage statement and insurance coverage. If in an HOA, documents showing what your monthly assessments are. Bank statements to show receipt of pay and access to any money needed to close the escrow on the refinance. They will also need Government issued ID, like a Drivers' License or Passport. Other unique issues may add additional items to the required list.
